Prior to your first appointment, please print and complete your first visit paperwork. Completing your paperwork before arriving helps us make the most of your first visit and saves you time.
Prepare for Your First Visit
- Make a list of all your current medicines and nutritional supplements, their doses, and how often you take them, or bring in the bottles.
- Make a list of all your past surgeries, hospitalizations and major illnesses.
- Bring your insurance card and a photo ID (driver’s license or passport).
- Payment is required at time of service so please be prepared to provide your co-pay or deductible.
- Write down a list of your questions.
- Bring paper and pen to take notes.
- Consider bringing a family member or friend, and ask them to take notes.
